The job
The Nurse Manager - Community Care is primarily responsible for coordinating and implementing the Community Care program for Veterans. This role ensures high-quality nursing practices in the referral, implementation and monitoring of patient care as Veterans move through the program. The Nurse Manager uses analytical frameworks, such as the nursing process, to create an environment that supports the delivery of non-medical care & collaborates with multiple disciplines and community vendors.
